Dental question for those who really understand Dental Insurance

For those who really understand Dental plans and the insurance game.
Received a claim back from the the insurance company that denied a dental procedure, basically a filling redone.

So who are the bigger of the ripoff artists, the dentist or the insurance companies? Opinions welcome from those with experience.

They might not have coded it correctly or sent the X-rays.
The dental office can try resubmitting.

This. At least talk to them and find out WTF happened. If it was required, then denying it makes no sense. Not paying due to deductible, UCR, etc. maybe, but denied? Nope.

This happens all the time. Try calling the insurance and the office. And yes, was it a matter of not meeting deductible or maxing out for the year, etc.?
